To use :

Write name on paper on one side on other side write their chosen fate

Light  candle once per night, let burn for 30mins while u envision their total annihilation,  and chant your chosen repeating words

do nightly until candle is burned right down.. 

On the last night before snuffing out candle out the paper w the name in the melted wax while you chant your incantation  at the end
